Patient Acknowledgements

I acknowledge that GetItPrescribed is not a physician, pharmacy, or healthcare provider.

GetItPrescribed provides administrative, management, and technology services to support independently contracted clinicians in delivering telehealth services. All medical decisions and care are provided solely by licensed healthcare professionals, not by GetItPrescribed.

I authorize GetItPrescribed to release my medical records to any licensed healthcare professional with whom I schedule an appointment through the GetItPrescribed platform, for purposes of treatment, continuity of care, and clinical decision-making.

This authorization supplements what is already allowed under HIPAA but is included here for clarity and transparency.

I understand and agree that GETITPRESCRIBED DOES NOT PROVIDE MEDICAL CARE, PHARMACY SERVICES, OR LABORATORY SERVICES.

I understand that GetItPrescribed is not responsible for the medical, pharmaceutical, or laboratory services I receive from independent clinicians, pharmacies, or laboratories.

I expressly waive any right to bring a claim against GetItPrescribed for services provided by independent healthcare professionals, pharmacies, or laboratories.

GetItPrescribed performs initial screening to verify that affiliated clinicians hold appropriate licenses; however, GETITPRESCRIBED DOES NOT GUARANTEE THAT PROVIDERS REMAIN CONTINUOUSLY LICENSED. Providers are solely responsible for ensuring they are properly licensed at all times before rendering care.

I understand that if a treating provider recommends a medication, the provider may send the prescription to a pharmacy known to be able to fill and ship that medication, particularly for compounded formulations.

I may request that the prescription be sent to my preferred pharmacy instead, but I acknowledge that not all pharmacies have the capability to fill all medications—especially compounded medications.

I agree that any dispute arising from my use of the GetItPrescribed platform or services will be resolved through binding arbitration, rather than through litigation or a jury trial.

This includes disputes related to telehealth services, platform use, payments, and interactions with affiliated providers.
